We recommend that you check with your insurance company to ensure that Dr. Freeman and Dr. Russell are listed as providers for your specific carrier.

We are not participating providers for Medicaid.

Alternative Communications.  This form is for patients to complete if they would like allow us to communicate with other people about their health information.  Completing this form authorizes us to speak about your health to the persons you designate on the form, whether or not you are present.  When completed, this form conveys this authorization only to us at Oakview Internal Medicine, and not to other physicians or organizations. 

 Record Release  Please fill out this form if you would like records sent from another physican's office to us.  After completing it please send the form directly to the physicians who are to send records to our office.
